The student agrees that Personal Training Academy Pty Ltd may obtain a consumer credit report containing information about them from a credit reporting agency for the purpose of assessing their application for commercial credit. The student also agrees that Personal Training Academy Pty Ltd may exchange information about them with those credit providers named in a consumer credit report issued by a credit reporting agency for the purpose of assessing their application, to notify other credit providers of a default by them, to exchange information with other credit providers as to the status of this loan where the student is in default to other credit providers and/or to access their credit worthiness. The student understands that the information exchanged can include anything about their creditworthiness, credit standing, credit history or credit capacity that credit providers are allowed to exchange under the Privacy Act.

Statement – I understand that it is required to provide the Victorian Government, through the Department of Education and Early Childhood Development, with student and training activity data which may include information I provide in this enrolment form. Information is required to be provided in accordance with the Victorian VET Student Statistical Collection Guidelines. Personal Training Academy Pty Ltd is collecting the information in this form for the purpose of assessing my entitlement to Commonwealth assistance under the Higher Education Support Act 2003 and the allocation of a Commonwealth Higher Education Student Support Number (CHESSN) to me. Personal Training Academy Pty Ltd will disclose this information to the Commonwealth Department of Industry, Innovation, Science, Research and Tertiary Education (DIISRTE) for those purposes. DIISRTE will store the information securely in the Higher Education Information Management System. DIISRTE may disclose the information to the Australian Taxation Office.
